Cancellation & Rescheduling appointment policyPLEASE READ
Please be aware our office has a 72-hour cancellation policy. If you need to cancel or reschedule an appointment, please do so at least 72 hours prior. Any appointment canceled or rescheduled less than 72 hours will be charged the full appointment fee with the credit card on file. For all appointments scheduled at WIFM, a credit card on file is required. 
Leaving a message in the portal or phone system is not a form of cancellation. You can not cancel a Monday appointment on Friday becuase we can not fill the appointment over the weekend. It will need to be cancelled no later than the Wednesday prior.
Although you may receive reminder phone calls, texts, and emails from our office about appointments, these reminders are a courtesy only. It is your responsibility to remember your appointment date and time. To cancel or reschedule your  appointment: Please call our office at (920) 968-9311.

Medication Refills - PLEASE READ

Weight Loss (oral & injectable) - LDN- THYROID - ESTROGEN  -TESTOSTERONE - PROGESTERONE  

Please go to the questionnaire icon and select one or more medication questionnaires for each refill.

Your medication will not be refilled without this form. There is no need to send an additional message as the form will be sent to the nursing triage pool. If we have further questions we will reach out. Otherwise, we will refill & send a confirmation message to you via the portal. Supplements can be re-ordered via Fullscript or picked-up at the clinic.
